What is the pathophysiology f DIC?
Release of procoagulants into system caused widespread activation of clotting, leading to consumption of clotting factors and therefore increased risk of bleeding
What can cause DIC?
Sepsis
Trauma
MAlignancy
Major haemorrhage
What is the presentation of DIC?
Microvascular clots- chest pain, SOB, leg pain
Extensive bleeding from anywhere- haematuria, from puncture sites
Bruising
Multiorgan failure
What investigations are done for DIC?
FBC- low platelets
D dimer elevated
Coagulation- prolonged PT, APTT and bleeding time
What is the management of DIC?
Treat cause
Replacements- FFP, cryoprecipitate
Platelets <50= give platelets
What are some complications of DIC?
AKI Life threatening thrombosis of haemorrhage Hypovolaemic shock Haemothorax Gangrene
What is the prognosis of DIC?
Poor- 20-50% mortality
Sepsis associated with poor outcomes
