Diagnostic Radiology: Respiratory System Flashcards
1
Q
Case 1: Pneumothorax
- Sudden pleuritic chest pain
- Dyspnea
A
- ***Hyperlucent hemithorax —> No vascular lung markings
- ***Absence of lung outlines
- Larger intercostal space
- ***Mediastinal + Tracheal shift
- ***Flattening of Hemidiaphragm
- Slight depression of left heart border (if left pneumothorax)
2
Q
Case 2: Pneumonia
- Productive cough + Fever
A
- ***Increased density
- Hazy + Indistinct border
- ***Air bronchogram
- Segmental / Lobar distribution
3
Q
Case 3: Left lower lobe collapse
- Chronic smoker
- Cough + Blood stained sputum
A
- Dense triangular shadow behind the heart (Sail sign)
- ***Loss of clarity of L hemidiaphragm
- ***Displacement of left hilum downwards (pulled downwards) (usually left mid-hilar point higher than right)
- ***Increased lucency in rest of ipsilateral lung (compensate for collapse of 1 lobe)
4
Q
Case 4: Heart failure
A
- ***Cardiomegaly
- ***Bilateral perihilar haze
- ***Upper lobe venous diversion
- ***Pleural effusion
5
Q
Case 5: Bronchogenic carcinoma
A
- ***Thick-walled lung mass
- ***Central cavitation
- ***Air-fluid level within cavitation
- ***Volume loss with tethering of hemidiaphragm
6
Q
Case 6: Pulmonary secondaries to Osteogenic sarcoma
A
Multiple dense nodules of variable size (***Cannon ball lesions)
7
Q
Case 8: Usual interstitial pneumonitis from Rheumatoid arthritis
A
- Bilateral ***reticulonodular shadowing
- ***Loss of lower lung volumes
- Shaggy heart borders + diaphragms
CT:
- ***Ground glass opacity (reversible)
- ***Honey-combing (irreversible)
8
Q
Case 9: Pulmonary embolism
A
- ***Wedge-shaped peripheral consolidation
- ***Blunting of left CP angle
CT pulmonary angiogram:
- multiple emboli in pulmonary arteries with peripheral wedge shaped infarct
9
Q
Case 10: Neurofibroma
A
- Extrapulmonary mass (no cervicothoracic sign: posterior to trachea)
- Pleural based
- Paraspinal
- Vertebral / disc disease
- Neurogenic lesions
