Q: List your diagnostic considerations in order of importance and explain your rationale
A
1. Angina: (this woman has stress-induced, non-exertional chest pain)-recent evidence shows that women present with more subtle symptoms of CVD.
-She has cardiac risk factors of Hypertension, past smoking, preeclampsia, and family history.
2. Panic Attack: Stress-related symptoms
- flashback to death of father
- suggestive anxiety, chest pain, and diaphoresis (sweating)
3. GERD: Alcohol intake has recently increased
- Some reflux symtoms
- Symptoms not triggered by meals
- Does not report heartburn
4. Musculoskeletal chest wall pain:
- no history of chest pain triggered by movementof upper torso or related excercise
- No notion of chest wal tenderness
5. Dissecting aortic aneurysm:
- no asymmetry of blood pressure noted
- no history of pain in neck, hear, or back
- no abdominal finding of a pulatile mass or bruit is noted
