Diagnosis And Management Of Cancer Flashcards
Important questions about the patient and their tumour
The critical role of the MDT
Will it come out cleanly
Is it prognostically operable
Is the patient fit enough for it - operation treatment, testing
Why do we do multimodality care - surgery and chemo
What we can see can be removed
Can’t know if the disease and microscopically spread further around the primary or to another area
So small can’t remove any thing so chemo is used to target this
Why give chemotherapy
To improve the outcome of definitive local therapy
What is adjuvant chemo
Reduction in the risk of occurrence
What is Neo adjuvant chemo
Facilitation of local resection and impact on micrometastatic disease
Why give palliative chemo
To improve the quality and quantity of life in the advanced disease setting
What questions are asked of the patient in order chemo is the best for them
Need to do a fitness test on the patient to determine if chemo isn’t going to impact them so much
Need to weigh up the benefits and the risks
Quantity and quality are important no point in living longer if pain and bed ridden - QALY
Need to be asked due to the expense of chemo and its SE
What is stratified medicine
The right treatment for the right patient
Finding the golden key, the generic lesion to which the tumour is addicted - the patients oncogenic driver
Key new mechanism and treatment targets
Hormone
Patients own immune system
Mutations targets
Combatting resistance
