Diagnosis:
identify a problem to form a (NANDA approved) nursing diagnosis
Diagnosis ACTUAL:
identifying current existing problem
we identify the current problem so we can create a plan of action to treat it, such as repositioning the patient for comfort, pain medication, etc.
example: patient has 6/10 pain aftercoming back from abdominal surgery
Diagnosis POTENTIAL:
identifying potential problems the patient is at risk for
we identify the potential risk so we can take action and implement prophylactic measures, such as early ambulation, DVT prophylaxis (heparin, lovenox, SCD,s
example: patient is at risk for developing DVT after surgery
Diagnosis Planning:
(SMART)
Set SMART goals to solve problems
(S) pecific ( who and what): clearly define the goal that is trying to be obtained
(M) easurable (by how much): create milestones and achievements that are clearly measurable
(A) chieveable (how): make sure goals are attainable based on current situation, resources, and skill
(R) elevant (why): state why is this goal important & how does it benefit the overarching goals & values
(T) imely (when): create time-bound objectives that set a reasonable time for goal to be achieved

Diagnosis implementation:
put plan into action; aka “NURSING INTERVENTATIONS”
examples:
~ placing SCD’s on patient
~giving medications
~ education patient
Diagnosis evaluation:
~ determine outcomes of goals
~evaluate effectives, modify & assess for needed changes to plan
