Diadynamic Current Therapy Flashcards

1
Q

What is it used for ?

Can it be used for muscle stimulation?

A

Used for pain relief ONLY

NOT muscle stimulation although it may cause contractions (such as in MF mode)

Q

What is diadynamic current

A

A transcutaneous electrical stimulation current that has analgesic effect

Defined as pulsed rectified alternating sinusoidal current
- Monophasic
- Half-sine waveforms

Q

What are the 5 DD current waveforms

A

Fixed diaphase (DF)
Fixed mono phase (MF)

Q

Fixed diaphase DF

A

Full wave rectified sinusoidal current

Continuous series of 10ms pulses of 100 HZ with NO interpulse interval

USED FOR:
-pain relief ( with spasm )
- introductory current
- improve circulation

Q

Fixed monophase (MF)

A

Half wine rectified sinusoidal current

10 ms half sine waves with 10 ms pulse interval

Frequency 50 HZ

USED FOR:
- pain relief (without spasm)
- improve circulation

Q

Short period (CP)

A

MF and DF are applied alternatively for 1 second each

( 1 second 50 HZ of MF / 1 second 100 HZ of DF)

NO interval phase

USED FOR :
- pain relief
- increased resorption (for swelling)
- improve circulation

Q

Long period (LP)

A

2 MF currents applied that occupy each other’s intervals

The second MF is surged (alternating amplitude)

USED FOR:
- LONG LASTING pain relief (good for nerve pain)
- increased resorption (for swelling)
- improve circulation

A

Delivery of MF for fixed duration followed by equal duration of rest

13
Q

Why do we have to start with 2 minutes of DF

A

Hypothesis:

Application of LOW AMPLITUDE DC will decrease transmembrane nerve potential , minimizing the current amplitude associated with subsequent delivery of either diadynamic currents

Q

Physiological and therapeutic effects

A

Pain relief also due to:
- depolarization of sensory/motor nerve fibers
- gate theory
- improve circulation through pumping muscle action that removes irritant waste

Improve circulation due to:
- release of histamine-like substance = vasodilation
- altering autonomic activity = vasodilation

Decrease inflammation and swelling:
- increased local circulation
- change of cell membrane permeability

Muscle re-education
- stimulation of motor nerves BUT IS NOT USED FOR MUSCLE STRENGTHENING

16
Q

General contraindication (hint: 8)

A

Impaired mentation (epilepsy risk)

Cardiac pacemakers / defibrillators (electronic interference)

Over thorax ( affect heart function)

Over anterior cervical neck region (stimulating vagus and phrenic nerves and carotid sinuses causing hypotension and laryngeal spasm)

Over cranial area (brain function)

Over venous thrombosis (embolism )

Over superficial metal implants (overheating)

Over malignant area (metastasis)

17
Q

Local contraindications (hint: 4)

A

Impaired sensation (causing severe pain)

Over abdominal, pelvic, lumbar areas of pregnant or menstruating woman (inducing labor / hemorrhage)

Over damaged skin or open wound ( severe pain)

Over hemorrhagic areas ( enhances bleeding )

18
Q

Expected sensation for DF

A

Light vibration

(Muscle contractions only occur at high intensities)

19
Q

Expected sensation of MF

A

Strong vibration

Muscle contractions

20
Q

Expected sensation of CP and LP

A

Aggressive current (rapid alteration b/w DF and MF)

LP is less aggressive than CP

21
Q

Electrode placement

A
22
Q

Treatment duration

A

12 minutes

Always start with 2 minutes DF

23
Q

Polarity?

A

Yes

You can reverse the polarity to treat opposite side
Ex: medial and lateral knee pain

24
Q

Number of treatment sessions

A

6-8

25
Q

What nerves are targeted

A

C fibers
