What is acromegaly?
XS GH causing the overgrowth of bone and soft tissue
GH pathway
Anterior pituitary –> GH –> liver –> IGF-1 –> tissues
Acromegaly main causes?
progestins in dogs

Increased GH does what?
increases insulin resistance (causes DM, cats)
increases bone/cartilage/soft tissue proliferation
canine Acromegaly clinical signs?
stridor airway compromise - pharyngeal hyperplasia extra skin mammary tumors diabetes space between teeth gingival overgrowth
acromegaly clin path?
hyperglycemia and glycosuria in concurrently diabetic cats
elevated ALP and ALT
mild hyperP
high or low PCV
acromegaly dx?
IGF-1 assay
most common clinical finding in feline acromegaly?
insulin-resistant DM, but rarely KA because insulin –> liver –> decrease ketogenesis
feline acromegaly signs ?
PD/PU/polyphagia organ enlargements cardiomyopathy lameness CNS dysfunction in late stages diabetes
acromegaly treatment?
iatrogenic –> discontinue progesterone treatment
natural onset in dog –> spay (OHE)
pituitary tumor –> surgery or radiation
NO medical therapy
give insulin if diabetic
acromegaly prognosis?
dog- good
cat- poor (long term)
Diabetes insipidus (DI)
PD > 100ml/kg/day
PU > 90 ml/kg/day
ADH function?
ADH made in hypo –> PP –> kidney –> water retention
DI types ?
Pituitary (central) - ADH does not reach kidney
Nephrogenic - kidney not responsive to ADH
