Diabetes insipidus- 2 types
Cranial diabetes insipidus- Decreased ADH secretion from pituitary
Nephrogenic diabetes insipidus- renal insensitivity to ADH
Cranial causes of diabetes insipidus
Post head injury
Idiopathic
Craniopharyngiomas
Wolfram’s syndrome
Nephrogenic causes of diabetes insipidus
Genetic
Hypercalcaemia, hypokalaemia
Lithium
Tubulo-interstitial disease
Symptoms
Polyuria and polydipsia
High plasma osmolality, low urine osmolality.
Do a water deprivation test.
Treatment
Nephrogenic- thiazides, low salt and protein diet
Cranial- desmopressin
Syndrome of inappropriate ADH secretion -SIADH.
What is it?
What causes it?
Hyponatraemia 2nd to dilutional effects of excessive water retention.
Seen in SCLC, neurological events e.g. stroke, haemorrhage.
