Initial: 5 mg daily but lower to 2.5 if renal impaired
2.5-5 mg daily

Linagliptin (DPP-4 inhibitor)

Ozempic or rybelsus
Semaglutide (GLP-1 agonist)
Rybelsus: start w 3 mg daily for 30 days then increase to 7 mg daily –> if still not at goal increase to 14 mg
Ozempic: start w 0.25 mg once weekly x 4 weeks then 0.5 mg once weekly x 4 weeks then 1 mg once weekly (2 mg if needed)
Trulicity
Dulaglutide (GLP-1 agonist)
0.75 mg sc once weekly, may increase to 1.5 mg
Victoza or saxenda
Liraglutide (GLP-1 agonist)
0.6 mg sc daily for one week, then 1.2 mg sc daily for one week, then 1.8 mg daily

Canagliflozin (SGLT-2 inhibitor)
100 mg po daily before 1st meal (100, 300) then 300 mg/day if needed
100-300 mg/day but max 100 mg if GFR <60
Farxiga
Dapagliflozin (SGLT-2 inhibitor)
5 mg (5,10) po daily in morning
5-10 mg daily
Jardiance
Empagliflozin (SGLT-2 inhibitor)
10 mg (10,25) once daily in the morning then 25 mg/d if needed
10-25 mg daily
Amaryl
Glimepiride (sulfonylurea)
Initial: 1-2 mg/day (esp in elderly)
1-8 mg daily (1, 2, 4mg)
