what is the dose of metformin?
500mg initially once daily for atleast a week - taken with breakfast.
THEN increased 500mg TWICE daily for atleast a week - taken with breakfast and evening meal
THEN increased 500mg THREES TIMES daily with each meal
MAX 2g daily
what can you combine metforming with?
- a sulfonylurea
- Pioglitazone;
- dipeptidylpeptidase-4 inhibitor
- a sodium glucose co-transporter 2 inhibitor only when sulfonylureas are contra-indicated or not tolerated, or if the patient is at significant risk of hypoglycaemia or its consequences.
what is the dose of sulfonylureas?
Glipizide Initially 2.5–5 mg daily, adjusted according to response (max 20mg)
what is the dose of meflitonides?
Repaglinide 0.5-4mg with meals
Nateglinide 60-120mg with meals
what is the dose of DPP 4 inhibitors?
Linagliptin 5 mg once daily
what is the dose of GLP 4 inhibitors?
For Saxenda®
Initially 0.6 mg once daily, then increased in steps of 0.6 mg, dose to be increased at intervals of at least 1 week; consider discontinuation if escalation to the next dose is not tolerated for 2 consecutive weeks. Discontinue if at least 5% of initial body-weight has not been lost after 12 weeks at maximum dose; maximum 3 mg per day.
what is the dose for pioglitazone?
Initially 15–30 mg once daily, adjusted according to response to 45 mg once daily, in elderly patients, initiate with lowest possible dose and increase gradually; review treatment after 3–6 months and regularly thereafter.
