Explain to a DES daughter what her risks are and outline plan for management:
A
Increased risk of:
- Clear cell adenocarcinoma of vagina and cervix
- CIN and VAIN
- Vaginal adenosis
- Breast cancer
- Genital tract abnormalities: T-shaped uterine cavity, hypoplastic uterus, endometrial adhesions, cervical malformations
- Pregnancy complications: infertility, miscarriage, 2nd trimester miscarriage, PTB, ectopics, stillbirth
- Earlier age of menopause
Management:
- Annual gynae exam for life: spec/bimanual, colposcopy, cervical screening (4 quadrant smear test + hrHPV
- Pelvic imaging to document structural abnormalities
- Regular breast exam and routine mammogram
