Explain to a DES daughter what her risks are and outline plan for management:

A

Increased risk of:

  • Clear cell adenocarcinoma of vagina and cervix
  • CIN and VAIN
  • Vaginal adenosis
  • Breast cancer
  • Genital tract abnormalities: T-shaped uterine cavity, hypoplastic uterus, endometrial adhesions, cervical malformations
  • Pregnancy complications: infertility, miscarriage, 2nd trimester miscarriage, PTB, ectopics, stillbirth
  • Earlier age of menopause

Management:

  • Annual gynae exam for life: spec/bimanual, colposcopy, cervical screening (4 quadrant smear test + hrHPV
  • Pelvic imaging to document structural abnormalities
  • Regular breast exam and routine mammogram
