Entire neck, posterior cheek, temporal area

Shoulder area, clavicular area, upper scapular area

Deltoid area, anterior aspect of the entire arm to base of thumb

Anterior arm, radial side of hand, thumb and index finger
Elbow flexion & wrist extension

Lateral arm and forearm to index, long and ring finger

Medial arm and forearm to long, ring and little fingers
Thumb extension & wrist ulnar deviation

Triceps reflex - nerve root and associated nerve
C7 - radial
Patellar reflex/knee jerk- nerve root and associated nerve
L4 - femoral
Achilles Reflex - nerve root and associated nerve
