Maculae
Non-palpable and less than 1 cm
Patch
Non-palpable and greater than 1 cm
Papule
Palpable, circumscribed, solid and less than 1 cm
Plaque
Palpable, solid, and often a confluence of papules greater than 1 cm
Nodule
Palpable, circumscribed, solid and >1,<2 cm
Tumor
Palpable, circumscribed, solid, and greater than 2 cm
Wheat
Plateau-like elevation due to edema in the skin
Transient
Surrounded by macular erythema
Vesicle
Fluid filled blister less than 1 cm
Bulla
Fluid filled blister greater than 1 cm
Pustule
Similar to vesicles, but filled with purulent material
Scale
Masses of keratin, papulosquamous when with a papule/plaque
Crusts
Scab
Any combination of serum, pus, blood with inflammatory cells
Excoriation and Abrasion
Mechanical removal of the epidermis, papillary dermis
Excoriation are typically self induced
Fissure
Cracks and clefts which are very well defined and linear
Erosion
Loss of a portion of the epidermis
Can be from a ruptured bulla
