Isotretinoin
A
DC: Oral retinoids
I:
- Severe cystic acne
D:
- 0.5mg/kg initially, could increase to 1mg/kg after 4 weeks.
- Generally use for around 4-6 months
M:
- Improvement of symptoms
- Excessive dryness
- Contraceptive use
- Pregnancy status
- Lipid levels
- Mood changes
- Regular eye check
- LFT
C:
- Better absorbed with food
- Tell doctor if any nausea, headache, vision changes (include poor night vision and blurred vision) to your doctor.
- This medication will cause dry eyes, mouth, and lips. Use Vaseline, stay hydrated, and lubricating eye drops. Tell doctor if you’re unable to manage it or when your contact lens feels uncomfortable.
- Your acne may get worst before it gets better
- Avoid taking any vitamin A supplements
- Avoid waxing or dermabrasion when on this medication and for 6 months after as it could increase the risk of scarring.
- Protect your skin from sunlight with protective clothing or broadspectrum sunscreen. Avoid sunlamps and tanning beds.
- Tell your doctor if you notice any changes in mood.
- Do not donate blood while you’re on this medication and for 8 weeks after
- Ensure adequate contraception, do not fall pregnant while you’re on this medication and for 1 month after.
- Do not share medication with anyone else
- Lifestyle
PP:
- Could cause hypertriglyceridaemia
- Could alter glucose tolerance.
- Could increase chances for corneal opacities. May also increase cataracts and optic neuritis but causality is unclear.
