Wheal
Itchy, raised, transient area of skin with blanching and erythema
Vesicle
Small blister filled with fluid </= 1cm diameter
Bulla
Large blister filled with fluid > 1cm diameter
Pustule
Discrete, pus filled, raised lesion
Caused by bacteria on or in epidermis. Pus beneath cornified layer or within spinous layer of epidermis
Erosion
Loss of partial thickness of epidermis
Surface moist but doesn’t bleed; heals without scar formation; usually occurs from scraping of surface of skin
Ulcer
Loss/full thickness loss of epidermis and upper papillary dermis
Always heals with scar formation
Scale
Flat plate or flake of cornified layer of epidermis
Crust
Dried residue of skin exudates, such as serum, pus, or blood
