Scales
Heaped up accumulations of horny epithelium

Dried serum, blood or pus
Petechiae
Pinpoint red, pink or purple macules from leaking capillaries, NON-BLANCHING, < 2 mm
Purpura
Petechiae that have coalesced/accumulated, may or may not be palpable, NON-BLANCHING, > 2 mm
Telangiectasia
Foci of small, permanently dilated blood vessels, BLANCHABLE
Erosions
Open areas of skin resulting from loss of part or all of the epidermis
Ulcers
Result from loss of epidermis AND at least part of the dermis
Atrophy
Thinning of the skin, can appear dry and wrinkled
Scars
Areas of fibrosis that replace normal skin after injury
Urticaria
Erythematous, elevated lesions caused by localized edema
Linear configuration
Straight line
Annular configuration
Rings with central clearing
Nummular configuration
Circular or coin shaped
Targetoid configuration
Rings with central duskiness/darkness
