What do you want to check with the dentures on the cast?
Correct shade and mould that you expected?
Adequate extension into the sulci
Check for overbites and overjets
Are the teeth positioned where they should be?
Check the occlusion
Are the centre lines where they should be? Is the articulator pin on the table?
Wax trimmed and rounded borders
Post dam cut?
What should you look for when you’re trying in the dentures?
Try mandibular denture in first because if you put the maxillary denture in first then it can dislodge the other one.
Retention and stability
Base extensions
LIMBO
Position of teeth compared to the soft tissues
Vertical dimension FWS
Even contact in RCP
Speech
Aesthetics
What factor would make you think that the denture was over or under extended?
Under extended- move the soft tissues out the way- the denture will drop.
Over-extended- denture will be dislodged during function- i.e. from muscle attachments.
- Too much lip support will make the denture drop.
What aspects would you want to look for in LIMBO?
Lip support
Incisal plane
Midline
Buccal corridor
Occlusal plane
Where should the teeth be set for uppers and lowers?
Uppers- posterior teeth should be slightly buccal
Lowers- all teeth over the alveolar ridge
What aspects of speech should you assess?
Fricative sounds- F, S, V.
- Tell the patient to say the days of the week.
If the patient is making contact with the teeth during speech, what does this mean?
Lack of FWS- re-register the occlusion.
If the patient isn’t happy with the shade and mould, what would you do?
Choose again and get the lab to reset the new teeth,
If the midline is incorrect, what would you do?
Mark the new midline and get the lab to reset all the teeth to this midline.
If there are major issues with LIMBO, what would you do?
Re-register and send to lab for reset.
What would you do if the try in was over or under extended?
Majorly over or under extended- re-register the occlusion.
Minor over-extended- adjust the wax.
Minor under-extended- take a wash impression and re-try to proceed to insertion stage but do a reline at this stage and not deliver the denture.
If there is an occlusal error, what would you do?
Measure OVD, RVD and freeway space.
If OVD is correct and freeway space is correct- remove the lower teeth and replace with a wax rim and re-register the occlusion at an even contact.
- you can also add wax to the side that is not contacting and register it this way, without taking the teeth off.
If OVD is not correct- remove the teeth from the side that makes contact and add a wax rim- register at the correct OVD.
