It is a diagrammatic representation of the teeth showing all the surfaces
What is the purpose of dental charting?
- Quick reference
- Aid treatment planning
- Forensic odontological evidence
- Witness of fact
What are the consequences of poor dental charting?
- Incorrect treatment
- Damages to patient
- Medico-legal implications
- Financial implications
How long do we need to keep records for?
Ideally records should be kept indefinitely but at least for 11 years after the completion of treatment
How long should we to keep children records for?
Until the patient is 25 year olds or for 11 years after competition of treatment (whichever if longer)
If you are doing paper notes what colour pen should. you write in?
Black pen
What is an MOD restoration?
Mesio-occlusal-distal restoration
What is an MI restoration?
Mesio-incisal restoration
What is an OL restoration?
Occluso lingual restoartion
What does a black circle on a dental chart mean?
Restoration
How do you represent a missing tooth on a dental chart?
A straight black line though the tooth
How do you represent a tooth that has moved due to orthodontic movement?
arrow in the direction of movement
How do you indicate caries on a dental chart?
An an UNFILLED circe
How do you indicate if a restoration, crown or filling is unsound or carious and needs replacing
Draw an UNFILLED circle around the restoration
How do you represent a crown on a dental chart?
Cr in a box
How do you represent a tooth for extraction?
Diagonal line that continues outside the box
What is a MCC on a dental chart?
Metal ceramic crown
What is a PBC on a dental chart?
Porcelain bonded crown
What is a PC on a dental chart?
Porcelain crown
What is a PJC on a dental chart?
Porcelain jacket crown

A dentine bonded crown

A full veneer gold crown (full gold shell crown)
