Calculus Index
- CL1 = Light, 1/3 of tooth
- CL2 = Moderate
- CL3 = Severe, more than 2/3 of tooth
Gingivitis Index
- GI1 = Mild Inflammation, no bleed
- GI2 = Moderate Inflammation, bleeding
- GI3 = Severe Inflammation, ulceration and spontaneous bleeding
Tooth Furcation Index
- F1 = Probe extends up to half crown width
- F2 = Probe extends >1/2 width
- F3 = Probe extends all the way through
Tooth Mobility Index
- M1 = Moves 1 mm or less
- M2 = Moves 1-2 mm
- M3 = Moves >2 mm
Periodontal Disease Index
- PD1 = Stage 1
- PD2 = Stage 2
- PD3 = Stage 3
- PD4 = Stage 4

* always needs root canal

Crown of tooth has normal enamel next to area of missing enamel
* occurs during developement
* dentin takes up stain very easily
