Q

all investigations for dementia

A

cognitive tests
blood presssure, ECG and cholesterol - vascular RF
bloods - thyroid function, B12, folate and calcium - sometimes HIV and syphilis - reversibel casues
CT head - stroke and subdural haemorrhage - exlcude other pathology
MRI brain - medial temproal lobe atrophy in alzeihemers disease
SPECT perfusion scan - diagnostic uncertaintiy frontla lobe dementia
CT scan. lewy body
lumbar puncture - biomarkers in AD - a beta don adn tau up

11
Q

main features of alziehemers

A

amyloid plaques - beta
nurofibrillary tangles - hyper-phosphorylated tau is what makes these up

12
Q

what enzyme cuts amyloid precursor protein

A

alpha secretase

pathologically cut at 2 sites also by a gamma secretase complex which cuts in the transmembrane bit - ending up with 3 sections of this protein - 1 section the beta part forms the plaques

13
Q

what is TAU

A

in healthy neurones part of structure of neurones

amyloid metabolises wrong way - beta - cause plaque causing tau to be phsophroyltated and response from microglia cells around these plaques all causing neuronal cell loss and this - alzeihemers

14
Q

what trisomy linked to alzeihemers

A

downs syndrome

more common or earlier onset - APP gene expressed on chromosme 21 so more likely to get this

inhertied alziehmers - genes responsible - mutuations of APP or the enzymes that cut it up - presenilin 1,2

Q

what can we do for dementia

A

diangosis relieves anxiety and helps coping for families
social suppor t
financial advice
coping strategies
research
respite care

17
Q

what is the target of anti-dementia drugs such as donepezil, rivastiagmine and galantamine

A

ache

galatine least side effects

all have cholinergic side effects - vagus for exmaple
vagal side effects - increase vagal tone
bradycardia

18
Q

what drug to treat dementia aviaalble as patch if oral route no tolerated

A

rivastigmine

19
Q

memantine moa

A

glutamat antagonist - mdma receptors

20
Q

LEWY body tx

A

cholinesterase inhibotrs work well
dopmaine agonst for parkinsonian features but can wosen pyshcoissi so balance
clonazepam good for REM probems
avoid neuroleptics for th ehallucinations adn pyschosis as it will worsen the motor sysmtpoms

21
Q

thrombocytopenia, haemolysis and impaired renal function. Lab findings reveal haematuria and proteinuria - hus what toxin

A

shiga
