what is the aim of this visit
patient leaves the surgery with a denture that is good and accurate fit, is comfortable, they are able to insert and remove, they know how to clean
what understanding must the patient have before leaving?
what to expect when wearing a new denture, what the risks are if the denture is not maintained
what do you do whilst the denture is still on the articulator?
check design again is as requested, examine dentures for roughness/blebs, check it seats properly, is the pin on the table, does the occlusion look correct, are any teeth on the cast broken?
what do you think about when removing the dentures from the casts?
do not use a lot of force as this should not be required and may damage components, think about path of insertion and removal
what do you do before you insert the denture?
with a gloved finger feel for lumps, bumps or rough sharp areas, remove excess acrylic with acrylic bur in a straight hand piece, small localised adjustments
what do you do after you insert the denture?
listen to the patient, pressure indicating paste or silicone applied to denture and ask patient to close into ICP, areas of pressure will be visible where there is no remaining paste, adjust the acrylic in these areas
what do you check when you insert the dentures?
fit, retention, stability, aesthetics, speech and occlusal contacts
what causes the initial speech alteration with a new denture?
excess saliva
what do you check with the fit of the denture?
that it is comfortable for the and the seats fully with no rests standing proud
what do you check with the retention?
the clasps are engaging the undercuts when seated
what do you check with the stability?
does it rock
what do you check with aesthetics?
incisal plane, shade, mould, lip support, smile
what do you check with occlusion?
do teeth meet in the prescribed occlusal scheme per the wax up, use thin articulating paper between the teeth to identify premature contacts, deflective contacts
what instructions do you give for the patient?
insertion/removal, coping with new dentures, pain, denture cleansing, speech, eating, refer to clinic patient information leaflet
