DVT
Formation of a clot in a large vein
Usually the LE
MC location for DVTs
Distal to popliteal vein
- Posterior tibial
What location of DVT poses greatest risk for embolization to the lungs
Proximal to Popliteal vein
- Femoral v.
- Iliac v.
Triad of RFs for venous clot formation
- Venous stasis - Immobility or proximal occlusion
- Damage to vascular endothelium
- Hypercoagulability state - inflammation, inherited, pregnancy, cancer, smoking
Pertinent +ves that suggest DVT
Thrombotic risk factors
- Recent surgery
- Pregnancy
- Cancer
= Prolonged bedrest or long-distance plane travel
SXS of DVT
- Asymptomatic
- U/l leg pain + swelling + warmth
Dx of DVT
Venous duplex ultrasound –> non-compressible venous lumen
What test can be used to exclude the diagnosis of DVT?
- Duplex ultrasound
- D-dimer
Tx of proximal DVT or concomitant PE
Anticoagulation (Warfarin, Heparin, LMWH, Factor Xa inhibitors)
Tx of DVT in pts. w/ high risk of bleeding or contraindication to anticoagulation
IVC filter–> Prevent PE
