Q

What metabolic mechanism generates heat in a newborn?

A

Brown fat - in newborns, disappears with age, rich in blood vessels and nerve endings. (Burn brown fat to generate heat)
When newborns experience cold environments — norepinephrine is released stimulating brown fat metabolism to break down triglycerides - cardiac output increases, increasing blood flow through the vessels of the
brown fat tissue = warmed blood because of the metabolic
activity of the brown fat

Q

What is phenylketonuria (PKU)?

A

Inherited metabolic disorder where newborn lacks the enzyme phenylamine hydroxylase. This enzyme converts phenylalanine (an essential amino acid) into tyrosine. Phenylalanine builds up in the blood and tissues causing cognitive impairment

Q

What is happening with coarctation of the aorta in a newborn?

A

Narrowing of the aorta usually at or near ductus arteriosus
• Obstructs blood flow
results in an increased pressure proximalf to the defect
• With a decreased pressure distal! to the obstruction
• Increased blood pressure in the arms with decreased BP in
the legs

Q

Why is baby orange or yellow understand
BONUS: indirect Coombs test

A

Test detects unexpected circulating antibodies in a woman’s
serum that could be harmful to the fetus.

If the test is positive, RhoGAM is of no value because isoimmunization has occurred. In this case, the fetus is carefully monitored for hemolytic disease.
