Deck 2 Flashcards
What should you do if the newborn is jittery and has a temperature of 97.2 Understand what to do if the newborn is jittery and has a Temp of 97.2.
• Put in islet warmer (radiant heat)
• Check glucose level
What metabolic mechanism generates heat in a newborn?
Brown fat - in newborns, disappears with age, rich in blood vessels and nerve endings. (Burn brown fat to generate heat)
When newborns experience cold environments — norepinephrine is released stimulating brown fat metabolism to break down triglycerides - cardiac output increases, increasing blood flow through the vessels of the
brown fat tissue = warmed blood because of the metabolic
activity of the brown fat
What does the newborn’s skin look like if they are postdating?
Baby born at 42 ½ weeks baby will have dry - cracked skin
Moro reflex
Allow head and trunk of newborn in a semi sitting position to fall backward to an angle of at least 30 degrees
• Newborn should symmetrically extend and then abduct arms at the elbows and fingers spread to form a “C”
What does a baby look like if they are post mature?(after 42wks gestation)
• Peeling, cracked, dry skin, leathery
• Long, thin body
• Meconium staining of fingernails and umbilical cord
• Long hair/nails
What do you do for a newborn who is small for their gestational age?
Monitor glucose levels
Your newborn is 12 hours post-delivery with a temperature of 37.5C/99.5F what is your priority concern?
Hyperthermia caused by increased heat production related to sepsis or decreased heat loss.
• Initiate immediate intervention
What is phenylketonuria (PKU)?
Inherited metabolic disorder where newborn lacks the enzyme phenylamine hydroxylase. This enzyme converts phenylalanine (an essential amino acid) into tyrosine. Phenylalanine builds up in the blood and tissues causing cognitive impairment
What do you want to do with a baby who has phenylketonuria (PKU)?
Formula low in phenylalanine.
• Mom limit intake of protein foods (meat, eggs, milk) - encourage peas, lettuce bananas, potatoes
What complications can a newborn have who has a mom with diabetes mellitus?
High risk for hypoglycemia
Hypoglycemia in baby can lead to what? What is the nurses priority for care?
Cyanosis and seizures prevention of hypoglycemia becomes a nurses
priority focus of care
What is happening with coarctation of the aorta in a newborn?
• Narrowing of the aorta usually at or near ductus arteriosus
• Obstructs blood flow
• results in an increased pressure proximalf to the defect
• With a decreased pressure distal! to the obstruction
• Increased blood pressure in the arms with decreased BP in
the legs
Why is baby orange or yellow understand
BONUS: indirect Coombs test
Test detects unexpected circulating antibodies in a woman’s
serum that could be harmful to the fetus.
If the test is positive, RhoGAM is of no value because isoimmunization has occurred. In this case, the fetus is carefully monitored for hemolytic disease.
How do you care for a newborn receiving
phototherapy?
Do not apply lotion
How to properly bottle feed?
Keep nipple filled with formula, DON’T ALLOW AIR
• Check the flow of the formula (not too fast or slow)
• Cradle newborn in semi-upright position - do NOT place supine (aspiration)
• Do best when held close at 45-degree angle
• Place nipple on the tongue
