What should operator position be for upper teeth?
Stand in front of patient.
Patients mouth at shoulder level
What should operator position be for lower teeth?
Left- in front , mouth elbow height
Right- behind, patient as low as possible
At what position should a child be seated for XLA?
30degrees to upright
What is the ideal technique for deciduous extractions?
Apically directed push Socket expanding movements Palatal/ lingual first Then buccal Support alveolus Removal always buccally
Describe technique for XLA of maxillary Anteriors
Straight deciduous forceps
Beak placed subgingivally
Screw driver movements
Describe technique for mandible Anteriors
Lower universal deciduous forceps
Beaks placed subgingivally
Screwdriver movement
Describe technique for maxillary molars
Upper deciduous molar forceps More socket expansion required Beaks into furcation area Pressure with palatal movement Buccal removal
Describe technique for mandibular molars
Lower deciduous molar forceps
Beaks into furcation area
Apical push with lingual movement
Buccal removal
What should be done immediately post XLA
Nip both sides of socket
Place bite pack for 5 minutes
Examine for haemostasis
Give post op instruction to parent and child
Describe post op instruction
Advise of haemorrhage control- bite down on hanky for 10 minutes if begins to bleed
If problems persist call surgery or a&e
Encourage good OH
avoidance of self inflicted injury re. LA
Advise of inappropriate food stuff
Avoidance of hot drinks
No rinsing or exercise for rest of day
Aspirin not appropriate for pain relief
What can be taken for pain relief for children
1-5 years 120-250mg paracetamol every 4-6 hours
6-12 years 250-500mg paracetamol every 4-6 hours
12-18 years 500mg paracetamol every 4-6 hours
