Cystic Fibrosis, Post-Surgery Patients Flashcards
Three most common pathogens affecting cystic fibrosis patients: (Pathogens, not diseases)
Pseudomonas aeruginosa
Haemophilus influenza
Staphylococcus aureus
Two less common pathogens affecting cystic fibrosis patients: (Pathogens, not diseases)
Aspergillosis
Mycobacterium avium complex (MAC)
Three common lung disease for cystic fibrosis patients: (Diseases, not pathogens)
Bronchiectasis, sinus infections, and nasal polyps
Use this test to confirm CF.
How many positive tests confirm CF? What level?
Sweat chloride test.
Two positive tests
Chloride level greater than 60 = CF
What’s the treatment for CF infection? How is it given?
Tobramycin given aerosolized or via PICC line
Chest physiotherapy for a CF patient incudes what three modalities?
Postural drainage and percussion
Chest wall compression (vest)
Intrapulmonary percussive ventilation (IPV)
What is malignant hyperthermia?
Rapidly increasing body temp caused by uncontrolled skeletal muscle metabolism leading to rhabdomyolysis and death.
How does malignant hyperthermia affect O2 consumption?
What other lab value is affected?
O2 consumption increases three fold
Blood lactate increases 15-20 fold
What should you do if you suspect malignant hyperthermia? x5
Discontinue anesthetics Hyperventilate with 100% FiO2 Sodium Bicarb Cool the patient with ice or cooling blanket, heat exchanger Monitor I&O's
Two things to remember about post-surgical patients:
Close observation to ensure good oxygenation & ventilation
Early ambulation is really good.
Name three anticholinesterase drugs used to counter NMBA.
Neostigmine, pyridostigmine, edrophonium
Name two anticholinergics used to counter NMBA’s
Atropine and glycopyrrolate
How do you treat post-op delirium?
What’s a side effect of this drug?
Haloperidol
Torsades de pointes
Post-op hypoxemia can lead to ___.
Cardiac ischemia
Malignant Hyperthermia is thought to be caused by a reaction to ___. The temperature will skyrocket! Also, ___ will get really high!
Anesthesia
Lactate
