Cushings Flashcards
Cushings Disease is______ of Addisons
Opposite
Cushings disease is _______ secretion of adrenal cortex hormones
Hypersecretion
- excess cortisol
- excess aldosterone
- excess androgens
testosterone & estrogen
-excessive ACTH
Besides Cushings Syndrome, how else can these hypersecrtion of hormones occur
- excessive use of corticosteroids
- adrenal or pituitary tumor
Cardiovascular Assessment of Cushings
Hypertension (from excessive aldosterone retailing Na and Water)
Tachycardia
Signs of CHF
Metabolic Assessment of Cushings
Increased Cortisol=Increased Fat metabolism
-Moon Face
-Buffalo Hump
Nutrition education
quit smoking
limit/avoid alcohol
low sodium diet
Integumentary S/S of Cushings
Striae (stretch marks) on abdomen and thighs
Easy brusing
Amenorrhea
Effects of Excessive Androgens
excess hair on face arms and back (hirtuism)
Acne
Menstrual irregulrites (oligomenorrhea)
Electrolyte Abnormalities of Cushings
(opposite Addisons)
-hypernatremia (from excess aldosterone)
-hypokalemia (too much Na = little K+)
-hypocalcemia (goes in same direction of K=)
-hyperglycemia (glucocorticoids increase glucose)
Management of Cushings
-Decrease of stop corticosteroid use
Remove tumor
Adrenal-ectomy (caution: can result in Addisons disease)
Monitor electrolytes and Cardio status
Risk for osteoporosis (from hypocalcemia=brittle bones)
Risk for infection (glucocorticoids decrease immune response)
Risk for skin breakdown (good skin care and wound care)
weigh client daily due to fluid overload
Nursing management for cardiac status of bushings patient
12 lead egg
Place patient on fluid______
Restriction
Medications for Symptoms
Antihypersentivies: for HTN
Diuretics: treats fluid retention
Nutrition education
quit smoking
limit/avoid alcohol
low sodium diet
Pathophysiology of Cushings
Pituitary gland secretes too much ACTH costing overproduction of cortisol
