What is sjorgens syndrome (2)
autoimmune condition characterized by lymphocytic infiltrates in exocrine organs
primary condition or can occur secondary to other autoimmune conditions like RA and SLE.
Symptoms (6)
dryness of the eyes and mouth (sicca symptoms).
arthralgia
fatigue
vaginal dryness
parotid gland swelling.
Diagnosis- antibodies + test (4)
based on the confirmation of ocular dryness (Schirmers test)= positive vital dye staining results
positive anti-Ro and anti-La antibodies
typical features on a lip gland biopsy.
Management- non pharma (3)
symptomatic
Lubricating eyedrops are used and saliva replacement products may be tried.
Regular dental care
Management - pharma (3)
Pilocarpine - stimulate saliva production but often causes side effects such a flushing.
Hydroxychloroquine -help with arthralgia and fatigue.
Other immunosuppression would usually only be used in the context of organ involvement e.g. interstitial lung disease.
