What is a never event
A serious, largely preventable, patient safety incident that should not occur if the a amiable preventative measures have been implemented by healthcare providers.
E.g. Wrong site surgery and retained foreign object post procedure
When are incidents considered ne’er events
- Known to have previously occurred and source of risk through reporting
- National guidance of safety measure that should have prevented occurring
- The occurrence can be identified, defined and measure in an ongoing basis
