When is intervention necessary?
A
- Primary/Secondary/Residual caries (left in a cavity)
- Failing restorations
- Tooth wear
- Trauma
- Developmental defects
- Endodontics

What factors lead to caries?
A
- Substrate, time, diet
- Demineralisation,
- Plaque retention, Susceptible sites
- Smooth surfaces/fissures
- Severity of attack/rate of progression
- Enamel, dentine, root caries (must be differentiates)
- Failure of restorations

When we view new caries?
A
- Look for signs & symptoms Prevention!
- Take history & examination
- Investigations (vitality/radiography)
- Diagnosis

What are options for dealing with caries?
A
- Prevention of further disease (Fluoride, Dietary advice, Cleaning)
- Watchful waiting (Frequency of recall is high)
- Filling without drilling!
- Surgical treatment (filling) – poor substitute for prevention though
- Referral

how can restoration failure occur?
A
• Recurrent caries
• Iatrogenic
• Restoration may break
Most important thing is pain

How does tooth wear occur?
A
• Erosion • Abrasion • Attrition • TSL Becomes a problem if -Patient is young -Happens quickly -Its progressive • TSL complains about - Aesthetics - Sharp edges of teeth on the tongue - May be pain – progression is usually slow

What are traumatic injuries?
A
- crown/root fracture
- Soft tissue injuries
- Head injuries send straight to A&E/Immediate referral

Is intervention necessary for developmental conditions?
A
- Intrinsic staining of teeth > fluorosis/drug-induced (tetracycline’s)
- Hereditary conditions >Amelogenesis imperfecta, dentinogenesis imperfecta
- may improve aesthetics & function

What happens when pulp dies?
A
may lead to acute abscess = bound within tooth –must open tooth where pulp is present (pulp is converted into pus) & oozes out,

When to refer?
A
- When diagnosed dental problem beyond level of ability
- When difficulty arriving at diagnosis (NO DIAGNOSIS = NO TREATMENT)
- Problem is systemic – as indicated from history/examination/physical examination
