How do we shape root canal?
- Shape root canal
- Drown in bleach
- Fill root canals and tooth
Problems with root canal ?
- Complexity of root canal system
- Curvature of root canals preventing us from shaping them and leading to procedural errors
- Difficulty in getting irrigants to all parts
What does radiographic assessment shows for root canal ?
- Are roots curved?
- How many canals are there?
- Is canal obvious or scleoresed?
- Is apex damaged or open?
What does a rubber dam do?
- Isolation
- mandatory
- Protects airway
- Prevents contamination by saliva
- so irrigants not swallowed
- Moisture tight seal – use oraseal if reqired
How to asses root canal?
- remove caries and restorative material And assess restorability
- If it can’t be clamped, it cant be filled
- If restorable, build back up to facilitate RCT
What is the access strategy for root canal?
- radiograph
- ## 11mm bur against radiograph
How to reorientate bur against pulp chamber ?
ENSURE THE TOOTH MEETS PULP, NOT COMES OUT OF THE ROOT
How to use the bur on pulp chamber ?
• through roof of pulp chamber and move to safe ended burs (Endo Z burs) – blunt bottom of the bur
How much pulp chamber is removed?
ALL until see the floor of the pulp
How do we chemo mechanical prep?
- make shape can irrigate & clean
* Filing = shaping, cleaning = irrigation
What techniques for ROOT CANAL?
stepdown technique
Stem winding motion too
Anti curvature filing- away from danger area
What the FILE types ?
- Stainless steel/Nickel titiantium
- K type (K-file, K-flex, K-flexo)
- H- type – very aggressive and only used in a cutting motion
- Files are colour coded
What is the Step down Preparation ?
- reverse flare is easiest
- Prepare straight, coronal portion of canal first – coronal flare
- Don’t force files to apex
- Gates – burs and hestrom are best for doing this
- Start with gates 4, swap to gates 3, swap to gates 2
How are gates used ?
with a burnishing motion, pushed into canal passively (cuts on the outsroke)
What to do after coronal flare?
irrigate – every time you take an instrument out irrigate
