crush injuries of hand Flashcards
what is crush injury?
A crush injury occurs when force or pressure is put on a body part. This type of injury most often happens when part of the body is squeezed between two heavy objects
causes?
industrial fall from height road traffic accident blast or bomb injuries bullet/ gunshots/chopper burns
classification of crush of injury (hand)
1. mechanism •machine press •ringer injury •degloving injury •saw or injury due to sharp edges •blast or bullet injuries
2.layer of involvement •skin •tendon •skeleton- stable,unstable,multiple fractures •isolated fracture •joint injury •nerves
level of injuries?
Zone v - below carpals area easy to manage
zone 1V- carpal tunnel area , chances of adhesion is more
zone 111 - metacarpals , excision of fascia is done
Zone 11 -bunnel’s no man’s area between distal palmar crease and base of proximal phalanx . minimal handling is indicated
zone 1 -beyond proximal phalanx, repair of edp needed immobilisation with k. wire is done for pip & mobilization for dip done
complications?
stiffness of hand VIC rapture of repaired structures adhesion/contracture Reflex Sympathetic Dystrophy (RSD) Syndrome carpal tunnel syndrome
what is zone of hand?
Zone 1 (distal interphalangeal [DIP] joint) Zone 2 (middle phalanx) Zone 3 (proximal interphalangeal [PIP] joint) Zone 4 (proximal phalanx) Zone 5 (metacarpophalangeal [MCP] joint) Zone 6 (dorsum of hand) Zone 7 (wrist) Zone 8 (distal forearm) Zone 9 (proximal forearm)
what is degloving injury?
degloving injury is a type of avulsion in which an extensive section of skin is completely torn off the underlying tissue, severing its blood supply
What is a ring avulsion injury?
A ring avulsion injury is sustained by a sudden force pulling a ring from a finger. This can result in severe injury ranging from circumferential soft-tissue laceration to complete amputation.
