Mouth to anus - but esp terminal ileum
What are the macroscopic changes?
Skip lesions
Strictures
Microscopic changes?
Inflammation - transmural
Ulceration - thin and deep = cobblestone mucosa
Fistula’s and granulomas
What will be seen pathologically that differentiates this from UC?
Fistulas
Granulomas
Peri-anal disease
Symptoms?
Abdo pain
Diarrhoea with no blood (usually)
Weight loss/anorexia
—> malaise and malabsorptive signs such as vit. deficiencies and anemia
Types of investigations?
Bloods
Imaging
Stools
What will be seen in bloods?
Raised CRP/ESR Raised WCC Raised Platelets Low Hb Low Albumin
What is stool tested for?
Calprotectin
and to test for campylobactor/c. diff
What imaging is done and why?
AxR - obstructions
CxR - perforations
MRI - assess severity
Endoscopy - for a biopsy
1st line treatment to induce remisson?
Ileocaecal = Budesonide (Steroid) Colitis = Sulfasalazine/5-ASAs
2nd line?
Prednisolone
3rd line?
Methotrexate
4th line?
Biologics like infliximab
