What is Crigler Najjar?
Poor conversion and clearance of bilirubin from the body.
UGT1 A1 defect.
What are the symptoms of Crigler Najajar?
Baby
Diarrhoea/ vomiting/ swallowing problems
Fever
Confusion/ Slurring/ Seizures
What is the treatment of Crigler Najjar?
Phototherapy/ Exchange transfusion
What are the causes of hyperkalaemia?
Renal Impairment
Drugs
Addisons (+low aldosterone)
Cell release
What is the treatment of hyperkalaemia?
10 ml 10% calcium gluconate
50 ml 50% dextrose + 10 units of insulin
Nebulized salbutamol
Treat the underlying cause
What is the treatment of severe hypokalaemia?
IV potassium chloride
Maximum rate 10 mmol per hour
Rates > 20 mmol per hour are highly irritating to peripheral veins
What is the treatment of hypercalcaemia?
Fluids
Pamidronate
What is the treatment of hypocalcaemia?
Depends on cause
Vitamin D
Calcium supplements
Surgery etc.
What causes cottage cheese like discharge?
Thrush
What causes green frothy discharge?
Trichomonas Vaginalis
What causes a strawberry cervix?
Trichomonas Vaginalis
What causes a thin grey fishy discharge?
Bacterial Vaginosis
What causes cervicitis?
Chlamydia
Gonorrhoea
How do you treat thrush?
Fluconazole 150mg (PO, STAT) (BUT NOT IF PREGNANT)
Clotrimazole 500mg (TOP, STAT pessary))
How do you treat trichomonas vaginalis?
Metronidazole PO 5-7 days
How do you treat Bacterial vaginosis?
Metronidazole PO 5-7 days
How do you treat Chlamydia?
Doxycycline (7 days) PO
-> Azithro 1g OD one day then 500mg OD for 2 days
How do you treat gonorrhoea?
IM ceftriaxone 1g (STAT)
How do you treat PID?
IM ceftriaxone
Oral doxycycline
Oral metronidazole
What is cluster A?
Odd Eccentric
What is cluster B?
Dramatic unpredictable
What is cluster C?
Anxious Fearful
