How do you test CN I?
A
Ask patient about changes in their sense of smell

How do you test CN II?
A
- Inspect pupils
- Visual acuity
- Direct and consensual pupillary light reflexes
- Accommodation reflex
- Colour vision (Ishihara chart)
- Visual inattention and visual fields (peripheral)

How do you test CN III, IV and VI?
A
Assess eye movements by drawing a big H, ask about double vision

How do you test CN V?
A
- Cotton wool / neuro-tip on each branch
- Assess muscle bulk of temporalis and masseter (clench jaw + open mouth against resistance)
- Jaw jerk reflex

How do you test CN VII?
A
- Facial expression (raise eyebrows, close eyes, bare teeth, purse lips, puff cheeks)
- Change in taste?
- Hearing louder than usual? (occurs in stapedius muscle paralysis)
- Inspect external ear

How do you test CN VIII?
A
- March on spot with eyes closed
- Whisper a word at changing distances
- Rinne’s test - tuning fork on mastoid process
- Weber’s test - tuning fork on centre of forehead

How do you test CN IX and X?
A
- Inspect uvula / soft palate as patient says “ah”
- Gag reflex
- Cough
- Swallow water and assess for delay

How do you test CN XI?
A
Assess trapezius (shrug shoulders) and sternocleidomastoid (turn head)

How do you test CN XII?
A
- Assess tongue appearance (fasciculations?)
- Protrude tongue (deviation?)
- Assess tongue strength (against cheek and press)
