What are the key components of a history and investigations of a pelvic mass?
Non specific symptoms
May have indigestion/early satiety/poor appetite
Altered bowel habit/pain
Bloating/discomfort/weight gain
Mass may be asymptomatic or causing pressure symptoms
How do you manage ovarian cancer?
Diagnosis may be made through: post operative, USS of abdomen and pelvis, CT, CA 125 (increased when there is disease within the perineum, can be benign or malignant)
Risk of malignance index: u (USS features) x m (menopausal status) x CA 125
Staging
1) ovaries with capsule intact
2) one or both ovaries with pelvic extension
3) one or both ovaries with peritoneal implants outside pelvis or + nodes
4) distant metastasis
Treatment
Surgery (laparotomy to stage and reduce disease bulk/clear)
Chemotherapy (adjuvant or neo adjuvant)
