PRECAUTIONS AND CONTRAINDICATIONS:
Uncontrolled seizures
Osteoporosis
Post-op
Uncontrolled Seizures Precautions:
Avoid high-intensity or maximal effort exercises that could trigger a seizure
Supervise closely during exercises to monitor for signs of an oncoming seizure
Ensure safety measures (e.g., use of padded equipment, avoiding environments where falls could lead to injury)
Uncontrolled Seizures Contraindications:
High-risk activities like heavy lifting or exercises where loss of control could cause injury (e.g., certain free-weight exercises)
Avoid exercising in water or other environments where seizures could lead to drowning or severe harm
Osteoporosis Precautions:
Focus on weight-bearing exercises, but avoid heavy loading, especially on the spine
Prioritize low-impact activities to reduce the risk of fractures
Monitor posture closely, avoiding exercises that involve flexion or twisting of the spine (which can increase fracture risk)
Osteoporosis Contraindications:
High-impact exercises (e.g., jumping or running)
Exercises with excessive spinal flexion or rotation (e.g., certain abdominal exercises, heavy lifting that strains the spine)
Post-Operative Precautions:
Follow surgical protocols for weight-bearing restrictions, movement limitations, and healing phases
Modify exercises to avoid stress on the surgical site until cleared by the surgeon
Use lower resistance and progress cautiously, especially with power exercises
Post-Operative Contraindications:
Avoid resistance training in the early post-op phase unless specifically advised by the medical team
No heavy lifting or high-resistance activities that might compromise the surgical repair
Avoid stretching or activities that place excessive tension on the healing tissue
1RM Testing:
Select Weight for Movement = guess a weight you think a child can successfully complete <5 times, then use calculator to estimate 1RM
adjust weight = child completed >5 reps of previous weight then increase weight/ child unable to complete a successful attempt then decrease weight
continue adjusting weight = child completes 1-5 reps, estimate 1RM/ child unable to complete repetition or completes >5 reps, then adjust
Functional 1RM Testing
choose your activity: Pushing, pulling, stairs, steps, walking
determine unresisted capacity: Distance/steps child can cover unweighted in 25 seconds
determine load: Add weight so the child can perform 50-70% of max effort (ie, distance, steps)
dose: Power training in short bursts of approx. 25 seconds, 6-8 reps
progress: Increase weight 10% when child can complete all reps in <25 seconds
GMFM-88:
Gross Motor Function Measure
standardized assessment used to evaluate gross motor function in children with cerebral palsy (CP) and other motor impairments
consists of 88 items divided into five dimensions:
Dimension A: Lying and Rolling
Dimension B: Sitting
Dimension C: Crawling and Kneeling
Dimension D: Standing
Dimension E: Walking, Running, and Jumping
GMFM-88 Dimension A Score (Lying and Rolling)
dimension includes 17 items, and each item is scored on a 4-point scale:
0 = Does not initiate the activity
1 = Initiates the activity (<10% completion)
2 = Partially completes the activity (10-99% completion)
3 = Completes the activity fully (100%)
maximum possible score for this dimension is 51
