How is the HPI different from the ROS?
The ROS includes all other information the pt has. It is rated in a positive and negative form.
Name five elements of the HPI
Onset, timing, location, quality, severity, modifying factors, associated sx, context.

Constitutional, eyes, ears/nose/throat, cardiovascular, respiratory, gastrointestinal, genitourinary, musculoskeletal, integumentary/skin, neurological, psychiatric, endocrine, hematologic/lymph, immunologic
Can the symptoms listed in the ROS ever contradict the systems described in the HPI?
No because they are both subjective coming from the pt’s POV
What do you need to remember to document in the HPI and ROS for any patient that is unconscious or incapable of providing information?
Document why the history is limited. “HPI unobtainable due to the pt’s non-verbal status”.
Why is it important to remember to document if the pt has had similar symptoms in the past?
It is likely that their current symptoms are related. It is also less likely that their current symptoms are life threatening if they have survived the same symptoms in the past.
Name one detail that is important to document if the pt has been evaluated in the past for a similar complaint.
Anything new or different about the sx today, how long ago their similar Sx occurred, if they sought professional treatment at the time, any result or dx form previous evaluations.

Documenting the answers to every question asked by the doctor.
