24 hour esophageal pH monitoring - most sensitive and specific test Ambulatory 24 hr esophageal pH monitoring- when pts do not respond to anti-acid therapy, or diagnosis other than GERD Antireflux surgery (fundoplication) - pts who do not respond to therapy and have documented positive esophageal pH monitoring
