Help prevent avoidable deaths.
1. Person has died
2. The persons identity
3. When and where the person died
4. The causes of death
5. The circumstance of death
Scumbie
S-uicde
C- clause unknown or no doctor certificate
U-natural or violent death
M- medical or surgical or dental
B- irth deaths or deaths that appear to be the result of pregnancy or giving birth
I- nstiutional deaths, that is deaths in a psychiatric hospital.
E- nactment requires holding of an inquest
Report deaths to coroner section 15
Coordinate a call to the NIIO National intial investigation office.
Post mortem section 31
Post mortem is authorised by a coroner and they direct a pathologist to perform and examination.
Coronial inquiry
- Fact a person died
- Identify of the person
- Cause of death
- Where and when of death
- Circumstance of death
Second phase of coronial inquiry
Drawn public attention to reduce the chances of the occurrence.
Third phase of the coronial inquiry
Whether public interest would be served by the death being investigated.
Inquest must be held if
Self inflicted
Any death that occur in official custody
Any death in which the coroner is not satisfied that the cause or circumstance of death was established clearly.
