Definition and causes of cord prolapse
A
- Descent of the umbilical cord through the cervix alongside (occult) or past the presenting part (overt) in the presence of ruptured membranes
- Main risk is asphyxia - ischaemic encephalopathy/cerebral palsy/death

Risk factors
A
- Malpresentation
- High PP
- Low placenta
- Prematurity
- Polyhydramnios
- Fetal anomalies
- Twins - second mainly

Management
A
- Help - 2222!
- VE - elevate the PP to relieve compression on the cord
- Stop synto if running
- Delay in delivery/home birth
- Turn mum onto all 4s
- Delivery usually caeserean (forceps if possible)
