Definition and causes of cord prolapse

A
  • Descent of the umbilical cord through the cervix alongside (occult) or past the presenting part (overt) in the presence of ruptured membranes
  • Main risk is asphyxia - ischaemic encephalopathy/cerebral palsy/death

  • Malpresentation
  • High PP
  • Low placenta
  • Prematurity
  • Polyhydramnios
  • Fetal anomalies
  • Twins - second mainly

Management

A
  • Help - 2222!
  • VE - elevate the PP to relieve compression on the cord
  • Stop synto if running
  • Delay in delivery/home birth
  • Turn mum onto all 4s
  • Delivery usually caeserean (forceps if possible)
