What is the definition of cord prolapse?
Umbilical cord comes out of the uterus with or before the presenting part of the baby
What factors are associated with cord prolapse?
Malpresentation
Preterm labour
2nd twin
Artificial membrane rupture
What is the pathophysiology of cord prolapse?
Direct compression and cord spasm leading to decreased blood flow and hypoxia and ultimately death
What are some clinical features of cord prolapse?
May have no outward physical signs and normal foetal heart trace
Abdominal exam can show ill-fitting or non-engaged presenting part
Vaginal exam can feel sometimes
What are some investigations for cord prolapse?
Can for foetal cardiac activity
- Prolonged and complete compression causes bradycardia
- Deteriorating foetal status causes activity to diminish and eventually stop
What is the management of cord prolapse?
Immediate delivery (c-section or forceps)
Tocolytic and maternal position to relieve pressure
