What is mode of action of COCP?
Inhibit ovulation
What are the UKMEC 4 Contraindications to COCP?
> 35 years old and smoking >15 cigarettes/day
migraine with aura
history of VTE
history of stroke/ IHD
breast feeding < 6 weeks post-partum
uncontrolled HTN
current breast cancer
major surgery with prolonged immobilisation
What are the UKMEC 3 Contraindications to COCP?
> 35 years old and smoking< 15 cigarettes/day
BMI > 35
FH VTE in first degree relatives < 45 yr
controlled hypertension
immobility e.g. wheel chair use
carrier of known gene mutations associated with breast cancer (e.g. BRCA1/BRCA2)
What are the advantages of COCP
Fertility returns to normal on stopping
Protection against colorectal, endometrial and ovarian cancer
Effect of COCP on periods?
Lighter, less painful
How to start COCP/ when will you be covered?
If started within 5 days of start of cycle then protected
Missed COCP 1 pill
If missed 1 pill at any time of the cycle, take the next asap even in 2 in same day
No further action required
Missed 2 or more COCP general advice
Use condoms or abstain from sex until at least 7 pills taken in a row
Missed 2 or more COCP 1st week (Days 1-7)
Emergency contraceptive if sex within pill free or 1st week
Missed 2 or more COCP 2nd week (Days 8-14)
After 7 consecutive doses of oral contraceptive pill there is no need for emergency contraception
Missed 2 or more COCP 3rd week (Days 15-21)
Finish pills in the pack and start the next one straight away (therefore omitting pill free week)
Risks/side effects COCP?
VTE, Stroke, IHD, Breast and cervical cancer

Thickens cervical mucus
Desogestrol inhibits ovulation
