Continuous Passive Motion
electrical motorized devices that move jts through a preset partial PROM at a preset controlled speed
What is goal of CPM?
prevent or decrease negative effects of immobilization
in theory it improves tissue healing
Most common uses of CPM
Acute/subacute: following ACL reconstruction,
following TKA, following osteochondral defects or chondromalacia
Shldr sx
Hand sx
Foot/ankle sx
What does constant, light stress applied to injured tissue do?
promotes more natural alignment of collagen fibers
How does CPM potentially help the ROM
may assist in gaining ROM early in rehab but no long term effects when compared to ROM exercises PROM to AROM best
May assist pt apprehension to ROM early in rehab since pt can predetermine ROM
How does CPM help with joint nutrition?
may assist in joint nutrition during immobilization following meniscal and articular cartilage repair–circulates synovial fluid
may be more effective than AROM
CPM and edema reduction
assists with venous and lymphatic return
CPM and pain reduction
gentle mvmt activates sensory afferents to control pain through gate control
What is main purpose to use CPM in regards to ligament healing?

Indications for CPM
TKA or other jt arthroplasty ACL reconstruction Osteochondral repair Chondromalacia surgery Tendon lacerations in the hand
Contraindications for CPM
unstable fx
conditions where CPM causes unwanted motion
uncontrolled infection
Set-up
measure length of thigh from ischial tuberosity to knee jt line (proximal part of machine to meet bottom of buttocks)
measure leg from jt lin to 1/4 in past heel
place extremity in unit and make sure the articular hinge of CPM unit is in line w/pts knee jt line
tibia in neutral position—no IR/ER to occur would place stress on ACL
