Q

What is this condition

A

Juvenile idiopathic arthritis

  • Swelling around DIPS
  • Juxta articular osteopaenia
  • Cortical thinning
  • Laminated periosteal response (can see a small line between the periosteal response and the cortex in the middle image on the ulnar aspect of the proximal phalanx of the 3rd digit

Q

What condition is this? Describe

A

DISH

  • Undulating contour ossifications
  • Preservation of disc space
  • ‘Candle flame’ osteophytes (maybe differentiate from OA by disc space preservation as osteophytes in OA are due to traction or increased shearing)

Q

What condition this is

A

DISH

  • flowing hyperostosis
  • undulating contour ossifications over numerous levels (has to be over 4+ levels to be diagnostic)

14
Q

What is this condition? What stage is it in? What are the signs of this stage?

A

AS

Stage 1

  • Bilateral and symmetric
  • Iliac side more extensively involved
  • Involves lower two thirds of joint.
  • Osteopaenia of the sacrum

Q

What is this condition? What stage is it? What can we see that indicates what this condition is?

A

AS

Stage 2

Articular erosions (rosary beads)

Diminished joint space

Loss of articular cortex definition (Pseudowidening)

Patchy reactive sclerosis

Subchondral osteoporosis

16
Q

What is this condition? What hallmark signs can we see in this image?

A

AS

Stage 3

Star sign - ossification of superior sacroiliac ligaments

Ghost sign - visualisation of articular cortex through ankylosed joint

Dagger sign - ossification of supraspinous ligs

Generalised osteopoenia

17
Q

What can we see in this image? What condition does it indicate?

A

AS

Rommanus lesions (black arrows)

Shiny corner sign - sclerosis in reaction to rommanus lesion (white arrow)

We can also see osteopoenia

18
Q

What condition is this?

A

AS

Uniform loss of joint space

Axial migration of femoral head

Protrusio acetabuli

Ankylosis of the joint

Erosion, periostitis at trochanters and adj ischial tuberosity caused by enthesopathy

Subchondral cysts

19
Q

What condition is this? Why?

A

AS

Dagger sign

Trolley tracks

Marginal syndesmophytes

Ghost sign

Star sign

Widespread osteopoenia

20
Q

What is this condition?

A

DISH

  • difficult to differentiate from AS, however DISH has paramarginal syndesmophytes in which the ALL is being ossified. We will often see a thin, radiolucent line between the vertebral body and the syndesmophyte if it is the ALL that is being ossified, where as in AS, it is the outer aspect of the annulus that is being ossified
21
Q

What is this condition? What is this hallmark sign called and what causes it?

A

AS

Bamboo spine - multiple marginal syndesmophytes

22
Q

What is this condition

A

AS

23
Q

What is this condition? What do we see?

A

PsA

At joint margins (fluffy Periosteal new bone formation) (mouse ears)

Whittled articular end of phalanx.

24
Q

What is this condition? What do we see?

A

PsA

At joint margins (fluffy Periosteal new bone formation) (mouse ears)

Progresses to whittled articular end of phalanx.

Whittled end erodes into adjacent cortical surface giving pencil and cup deformity.

25
Q

What is this condition? What do we see?

A

PsA

Joint margins (fluffy periosteal new bone formation) (mouse ears)

26
Q

What is this condition? What do we see?

A

PsA

Non-marginal syndesmophytes.

27
Q

What is this condition? What is the difference between the 2 images?

A

PsA

Non-marginal syndesmophytes.

Right: Complete: attached to mid body of two contiguous vertebrae.

Left: Incomplete: Comma shaped. (teardrop)

28
Q

What is this? what condition is it found in?

A

PsA

Non-marginal syndesmophyte

Floating: Unattached ossification bridging disc space.

29
Q

What is this? What condition is it found in?

A

PsA

Non-marginal syndesmophyte

Bagpipe: bulkier base attached to mid body

31
Q

What do the arrows indicate? What condition is this?

A

Cortical erosion, sclerosis, Periosteal “Whiskering” extending from bone into ligament or tendon.
