What is mastitis?
Infection/ Inflmmation of breast tissue
Which pathogen is the most common infective organisms in mastitis?
Staph Aureus
List some non-infective causes of mastitis…
- Duct ectasia
- Foerign material: piercing, breast implant, silicone
- Granulatomous mastitis
What happens in lactational mastitis?
In lactational mastitis, milk stasis or milk overproduction, coupled with infection from bacteria entering the breast via a traumatised nipple (e.g., cracked or fissured) and/or from the infant’s mouth, can lead to mastitis
List some risk factors for mastitis…
- BREAST FEEDING
- poor latch
- cracked nipples
- use of a breast pump
- weaning
- tight fitting clothes
- trauma
- post nipple piercing
- foreign body- implants, silicone
Which group of people are at a higher risk of non-lactational mastitis?
smokers
30-60years
List some of the presenting symptoms of mastitis
- flu-like symtoms- myalgia, malaise
- fever
- breast pain esp whilst feeding
- redness and swelling
- decreased milk outflow
List some of the signs of mastitis on physical examination
- tenderness
- firmness
- warmth
- swelling
- erythema
- cracked nipples, inversion
- pyrexia
List some appropriate investigatons to conduct on someone with mastitis
- Clinical examination
- Ultrasound- to differentiate between tumour and abscess
- Needle aspiration/ nipple discharge: Culture + MC&S to determine organism
How would you manage a patient with mastitis?
- CONSERVATIVE: effective milk removal, hot pack to relieve tenderness, paracetamol/ ibuprofen for pain
- MEDICAL: antibiotics (flucloxacillin/ clindamycin- anaerobes)
- SURGICAL: surgical incision/ percutaneous drainage of abscess
- Triple assessment of any residual masses
