MISC
Exemptions to the sworn affidavit requirement: A sworn affidavit is required except for situations when:
- Criminal conduct is alleged anonymously;
- A violation of the medical policy is alleged;
- A residency violation is alleged;
- the reporting party is a Department or COPA member; or
- There is a sworn affidavit override approved by either the Chief Administrator, COPA, or the Chief, BIA, as appropriate.
