Competency Check Flashcards
15 contraindications for e-stim
-electronic devices
-LB or abdomen of pregnant
-acupuncture points of pregnant
-malignancy
-active DVT or thrombophlebitis
-active bleeding, untx hemorrhagic disorders
-infected tissue
-radiated tissue
-chest with cardiac disease/arrhythmia/HF
-neck or head w/ seizures
-near reproductive organs w/o training
-near eyes
-anterior neck, carotid sinus
-damaged skin
2 contraindications for TENS
-impaired sensory
-cog/comm impairments
5 NMES contraindications
-prego
-impaired circulation
-unstable area d/t surgery, bone fx, osteoporosis
-chest, intercostal mus
-lower abdom
2 precautions for estim
-active epiphysis
-skin diseases
2 NMES precautions
-cog/comm impairment
-impaired sensation
2 TENS precautions
-impaired circulation
-anterior chest wall or lower abdom
why are the following contraindications for estim:
-electronic devices
-prego
-malignancy
-active DVT
-hemorrhagic condition
-active bleeding
-infected tissue
-radiated tissue
-chest w cardiac disease
-neck or head w/ seizures
-malfunction and can be life threatening
-unwanted uterine contractions
-may stimulate growth of malignancy
-cause thrombus to dislodge and lead to PE, stroke, organ failure
-cause uncontrolled bleeding
-spread infection
-stimulate growth of remaining malignant cells
-high current may cause arrhythmias or fibrillation
-may induce seizures
why are the following contraindications for estim:
-near reproductive
-near eyes
-anterior neck
-damaged skin
-risk for gametogenesis are not known
-risks unknown; no clinical indication
-may stimulate vagus or phrenic nerves, decrease BP, cause pt to faint
-cause pain?
Reasons for 2 TENS contraindications
-impaired sensory: may cause burn
-cog/comm impairment: risk of ineffective tx and increased risk of burn
reason for 5 NMES contraindications
-prego: unwanted uterine contractions
-impaired circulation: increase risk of burn
-unstable area: strong contraction can cause fx displacement, tear of tendon or skin, disrupt stapes or incision site
-chest/intercostal: chest cause arrhythmias, intercostals can affect breathing
-lower abdomen: may cause ischemic colitis (decreased BF to lg intestine cause inflam)
reasons for 2 estim precautions
-active epiphysis: risk of abnorm bone development
-skin diseases: could stimulate inflam processes, placing pad on can cause tissue loss
Reason for 2 NMES precautions
-cog/comm impairment: ineffective tx, increased risk of burn
-impaired sensation: risk of burn
Reasons for 2 TENS precautions
-impaired circulation: risk of burn
-anterior chest wall or lower abdomen: chest arrhythmias, lower abdomen ischemic colitis
What is monopolar placement used for (4)
-ionto
-acute edema
-dermal ulcer wound
-small mus group or tx area
What is bipolar placement used for (5)
-disuse atroph
-NM facilitation to increase mus force
-ROM
-spasms
-chronic circulation disorders
What is quadrupolar placement used for (2)
Large area or stimulate deep tissues
what are the names of the 6 TENS protocols
-conventional
-strong low rate
-brief intense
-pulse burst
-modulated
-hyperstimulation (noxious)
What is the frequency, amplitude, and duration for conventional
high frequency: 50-100 pps
low amplitude: tingle, no mus contraction
short pulse: 50-100 µsec
what is conventional TENS used for (4ish)
acute, chronic and post-op pain
high-end sensory stim for painful procedures (ex. transverse friction massage, wound debridement, joint mob)
What is the frequency, amplitude, and duration for strong low rate
low frequency: 2-4 pps
high amplitude: about 40 mA
-visible rhythmic mus contraction, uncomfy but tolerable
long pulse durations: >150 µsec
what is strong low rate used for
deep, aching, chronic pain
What is the frequency, amplitude, and duration for brief intense
high frequency: 60 - >150 pps
high amplitude: about 40mA
pulse duration: 50-250
What is brief intense used for
therapeutic procedures (jt mob, contract relax stx, debridement, suture removal)
What is the frequency, amplitude, and duration for pulse-burst
High carrier frequency: 60-100 pps
Modulated low burst frequency: 0.5-4 pps
Pulse duration: 50-200
