What is NHS continuing healthcare?
A
- This is a fully funded package of care that people are entitled to receive as a result of disability, accident or illness
- It covers the full cost of the person’s care and residential accommodation

What is Discharge to Assess Bed?
A
- Where people who are clinically optimised and do not require an acute hospital bed but may still require care services are provided with short term, funded support such as a stay in a nursing home.
- You will have ongoing physiotherapy and support whilst at the care home

What is nursing home care?
A
- The purpose is to provide care and support for people with specific conditions
- With at least one registered nurse on site at any time it ensures residents have access to 24 hour medical care and nursing support
- Regular visits from GPs, Physiotherapists and other experts

- Residential care is used to describe the general care and support provided in a standard elderly care home
- Often referred to as personal care or assisted living
- Involves help with basic needs: washing, dressing and mobility assistance

- Intermediate care services provide support for a short time to help you recover and increase your independence
- This type of care is known as reablement that helps you relearn how to do daily activities such as cooking and washing

- They will help with all basic needs of the patient
