- Clearly introduce yourself using appropriate credentials
- Ask if the health care provider has time to talk
- ID the patient
- Utilize SBAR to communication information.
- Allow health care member to respond
- Dialogue to reach a mutually agreeable plan
- SUMMARIZE THAT PLAN
- Conclude by thanking the individual

A
Situation: What is the situation OR what is going on with the patient?
Background: what is the background of the patient or the context?
Assessment: What do you think the problem is? If you do not know, it is OK to state that as well.
Recommendation: what do I recommend/request be done?
In a red flag situation, stating what you are doing. You do not need feedback to do it. You are just updating them.
