Preinteraction phase
-occurs before meeting the patient
-review available data including medical and nursing history
-talk to other caregivers who have information about the pt
Orientation Phase
-when the nurse and the patient meet and get to know each other
-sets the tone for the relationship by adopting warm, empathetic, carring manner
-begin to make inferences and form judgments about pt messages and behavior
-assess pt health status
-prioritize pt problems and identify their goals
Working Phase
-when the nurse and the patient work together to solve problems and accomplish goals
-encourage and help patient with self-exploration
-provide information needed to understand and change behavior
-encourage and help pt set goals
-take actions to meet goals set with pt
Termination Phase
during the ending of the relationship
-remind patient termination is near
-evaluate goal achievement
-separate from pt by relinquishing responsibility for pt care (shift change)
-achieve a smooth transition for the pt to other caregivers as needed
